About The Position

It's more than a career, it's a calling. The position involves providing care to patients under the direction of a registered nurse or physician, functioning within the scope of license. The role is based at the SSM Health Hospice House in Baraboo, Wisconsin.

Requirements

• Graduate of an accredited school of nursing or educational equivalency for licensing.
• One year licensed practical nurse experience.
• State of Work Location: Illinois Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA).
• Driver’s License: State of Issuance and Driving Record - Motor Vehicle Report (MVR).
• Licensed Practical Nurse (LPN) - Illinois Department of Financial and Professional Regulation (IDFPR).
• State of Work Location: Missouri, Oklahoma, Wisconsin Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA).
• Licensed Practical Nurse (LPN) Nurse Licensure Issued by Compact State or Licensed Practical Nurse (LPN) - Missouri Division of Professional Registration.

Responsibilities

• Delivers care to assigned clients and performs direct care in accordance with an established plan of care.
• Delivers safe care at all times that is in line with physician orders, home health policy and established nursing care standards.
• Maintains communication regarding patient progress with physician, supervisor, staff and family members as appropriate to the situation.
• Reports patient-related observations to supervising RN on a daily basis.
• Administers medications and observes patients for adverse reactions to medications or treatments.
• Provides documentation that follows the established treatment plan, supports coordination of patient care, meets regulatory requirements and ensures reimbursement.
• Works in a constant state of alertness and safe manner.
• May perform point of care testing according to policies and procedures.
• Performs other duties as assigned.
