About The Position

The Community Hospice has a rewarding opportunity for an LPN to work as part of our team caring for patients and families in their homes primarily in the Saratoga/Washington county area. This position is full time, weekdays from 8am to 4pm, with no weekends and no mandatory overtime. The role involves coordinating closely with our RN Supervisor and RNs to provide nursing and personal care services as part of a comprehensive plan of care to support patients and families. Training will be provided in end of life care.

Requirements

• Valid NYS LPN License
• Minimum 1 year LPN experience required
• Strong computer skills
• Organization and communication skills
• Ability to work as part of a team

Nice To Haves

• Prior hospice/home care experience preferred, but not required

Responsibilities

• Coordinate closely with RN Supervisor and RNs to provide nursing and personal care services.
• Support patients and families as part of a comprehensive plan of care.
• Work as part of a team that includes RNs, Aides, medical directors, social workers, chaplains, complementary therapists, and volunteers.
