The Licensed Practical Nurse (LPN) participates in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting. Requirements

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
• Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
• Obtains and subsequently maintains required department specific competencies and certifications.
• Basic Life Support (BLS) for health care providers required.
• One year of professional nursing experience preferred.

Nice To Haves

• Experience in hospice or end-of-life care.
• Knowledge of medications and their effects on patients/residents.
• Knowledge of patient/residents education principles to develop a self-care program.

Responsibilities

• Demonstrates knowledge of nursing principles for the patient/resident in the relevant setting.
• Possesses the ability to perform testing to assess data reflective of the patient's/resident's status.
• Interprets appropriately the information needed to identify each patient's/resident's requirements relative to age-specific needs.
• Ensures the health, comfort, and safety of patients/residents.
• Demonstrates competency and practices within the full scope of nursing expertise/knowledge.
• Utilizes appropriate age and population specific standards as designated in the home care setting.
• Administers a variety of treatments and medications as directed.
