About The Position

We are hiring a Licensed Practical Nurse (LPN) to provide Behavioral Health and community-based healthcare visits in East Haven. This part-time, PRN position requires visits every other weekend, focusing on 1:1 patient care. Requirements

• Current LPN licensure in the state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance.
• Access to a dependable vehicle or public transportation.

Nice To Haves

• Previous home care experience preferred.

Responsibilities

• Provide direct nursing care as prescribed by the physician to acutely and chronically ill patients in their homes.
• Document patient visits thoroughly in the electronic medical record.
• Coordinate patient care activities and services to ensure high-quality patient care.
• Participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition.
• Ensure confidentiality of patient health information.
