About The Position

The Licensed Practical Nurse (LPN) provides skilled nursing care under the direct supervision of the Registered Nurse. Performs selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care. Is responsible and accountable for making decisions that are based on the individual's nursing experience and educational preparation. Assists th e Registered Nurse in carrying out the Plan of Care. Assists the Registered Nurse in performing specialized p r ocedu r es as directed by the Registered Nurse und e r physician ' s orders. Assists with the p r eparation, i m ple m entation and on-going ev a luation of the client care plan. Recognizes and understands the effects of social and econo m ic proble m s upon clients and provides for the e m otional and phys i cal co m f ort and safety of clients. Helps teach the patient the appropriate self-care techniques. F o sters cooperative effort a m ong person n el by understand i ng t h e functions of other persons involved in client care and by acti v ely participating in team and staff conferences. Assu m es responsibi l ity for personal and vocational g r ow t h and develop m ent through active participation in nursing organizations and in-service progra m s and other on the job learning. Enters clinical/progress notes (not to include ad m ission) into EMR within 24 hours of scheduled visit. Observes, records, and reports to the app r opriate person the patient’s sy m p to ms , reactions and changes includ i ng: The g e n e ral physical and mental condition of clients and signs and sympto m s that m ay be ind i cative of untoward c h a nges. Stresses in hu m an relationships between the client and staff personnel and between client and fa m ily and visitors. Reports any abnormality in the client’s range of m o tion, body m ech a n ics or body align m ent. Maintains current TB Testing (refer to Health Screening Policy). Is knowledgeable of the Agency’s organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time of requests. A Licensed Practical Nurse with the edu c ation and proven clinical competence to perform certain treatments/procedures may do so provided that the Home Health Age n c y ’s po l icies and the State’s Regulatory Acts allow such performanc e . Only advanced LPNs with documented skills may perform the following: A d m inister m edications Hang hydrating flu i d without m edications Flu s h p e ripheral lines with saline or heparin s o lutions Calculate and ad j u st flow rates Hang I. V .s pre m ixed with m edications Start peripheral I.V.s Inspect insertion sites, change dressings, and re mo ve peripheral I.V. needl e s/catheters C h eck b l ood wi t h a Registered Nurse

Requirements

  • Licensed Practical Nurse with the edu c ation and proven clinical competence to perform certain treatments/procedures may do so provided that the Home Health Age n c y ’s po l icies and the State’s Regulatory Acts allow such performanc e .
