The Licensed Practical Nurse HH assumes responsibility for the delivery of patient care utilizing the nursing process, under the supervision of a Registered Nurse and following the MD ordered plan of care. The position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires

Requirements

  • 1 year of LPN experience
  • LPN (Licensed Practical Nurse)
  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state.
  • BLS (Basic Life Support) Issued by American Red Cross or American Heart Association OR CPR (Cardiopulmonary Resuscitation) Issued by American Red Cross or American Heart Association within 30 days of hire.
  • Must be able to communicate effectively in English (verbal/written).
  • This job requires the incumbents to operate a INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.

Responsibilities

  • Performs patient assignments as coordinated with the Care Manager, Clinical Supervisor, and scheduler under the direction of the Administrator.
  • Performs specialized skills as per agency policies/procedures
  • Ensures implementation of interventions for improved quality of patient care and outcomes per established plan of care.
  • Reports changes in patient status to the provider and initiates any new orders from the provider as directed.
  • Reports clinical update to the RN Care Manager assigned to each patient after each visit.
  • Instructs the patient/caregiver in signs and symptoms of complications and home management related to disease processes as outlined in the established plan of care.
  • Assists the patient/caregiver in following medical recommendations.
  • Participates in dept quality improvement planning and development.
