About The Position

The Home Health LPN Provides skilled care services to assigned patients in their place of residence, under the supervision of a registered nurse. Actively participates in providing patient care as defined by multidisciplinary plan of care. Provides nursing care to patients from young adult to geriatric populations with diverse cultural backgrounds in accordance with home health policies and regulations.

Requirements

Active SC LPN license
At least one year of current, direct clinical experience clinical experience in a nursing home, hospital, or med/surg clinic required.
