About The Position

The Licensed Practical Nurse (LPN) - Home Health and Hospice (MI) provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) and in accordance with the plan of care. The LPN supports both the home health and hospice teams by assisting with medication administration, wound care, symptom management, and patient/caregiver education. This position requires independent judgment, effective communication skills, and the ability to deliver compassionate care in a variety of home environments. The LPN collaborates closely with the interdisciplinary team to ensure continuity of care, timely documentation, and compliance with regulatory and safety standards. Services are provided in accordance with Marshfield Health System policies, CMS regulations, and state nursing practice guidelines.

Requirements

• Completion of a recognized and duly approved practical nursing curriculum.
• Demonstrates clinical competence and working knowledge applicable to both Home Health and Hospice settings, including awareness of age-specific needs.
• Must possess the ability to cooperate and communicate effectively with peers, patients, families, physicians, and other healthcare professionals.
• Current registered Licensed Practical Nurse in the State of Michigan, and able to obtain Wisconsin license within 60 days of hire.
• BLS required within 30 days of hire.
• Must possess valid driver's license and reliable vehicle.
