About The Position

AuthoraCare Collective is seeking a Full-Time LPN Home Care professional for the Burlington service area. This role involves providing high-quality nursing services under the supervision of a Registered Nurse, ensuring compassionate care for patients with limited life expectancy. The LPN will work collaboratively with an interdisciplinary team to meet patient and family goals, while also participating in weekend and weeknight coverage as needed.

Requirements

• Must be a graduate of an accredited LPN program.
• Must have a current North Carolina LPN license.
• One year's LPN experience necessary, preferably two years in an acute care, hospice, LTC or home health setting.
• Valid state-issued driver's license required.
• Must carry automobile liability insurance at limits required by agency.
• Must have own transportation.

Nice To Haves

• Experience in teaching is preferred.

Responsibilities

• Provide high-quality nursing services under the direction of a Registered Nurse.
• Establish therapeutic relationships with patients and caregivers.
• Provide compassionate, basic personal care for patients with limited life expectancy.
• Administer medications as directed and document care.
• Work with other members of the interdisciplinary group to meet patient/family goals.
• Participate in providing weekend and weeknight coverage as assigned.
