About The Position

The Licensed Practical Visit Nurse (LPVN) is responsible for the provision of home care services for patients under the supervision of a registered nurse. The LPN visit nurse will provide care within their scope of practice as ordered by the physician, follow policies, procedures, Conditions of Participation, and regularly report to the supervisor. The incumbent will facilitate the implementation of the plan of care as outlined by the physician. In addition, the incumbent will be responsible for all required documentation related to the care delivered and will communicate pertinent information regarding the patient to all appropriate health care team members. Requirements

  • Minimum Level of Education Required: Associate degree Associate degree
  • Type of degree: Associates or Bachelor'sAssociate's or Bachelor's
  • Area of study or major: LPNLPN
  • Current Licensure (LPN) in the State of Ohio
  • CPR Certification
  • Minimum Level of Experience Required: No prior job-related work experience
  • Must be capable of standing, stooping, bending and lifting, up to 50lbs
  • Must be able to take direction and to work as a team member
