About The Position

OVERVIEW OF POSITION: As an integral member of the healthcare team, the LPN provides direct and indirect nursing care to patients as delegated by a Registered Nurse or provider. The LPN must adhere to organizational Policies, Procedures, the Code of Ethics, and all licensing and accreditation standards for the position. The LPN is expected to represent the organization’s Mission in all actions and communications. This is a part time 30 hours per week position typically working during normal clinic hours (typically Monday through Friday from 8 a.m. to 5 p.m.).

Requirements

  • BLS/CPR training according to license and/or certification requirements.
  • Understanding of the physical, emotional, and psychosocial needs of patients.
  • Effective communication and interpersonal skills.
  • Ability to be self-motivated to work with and serve a diverse population.
  • Ability to work effectively and efficiently under tight deadlines and high volumes.
  • Working knowledge of computers and their applications.
  • Associate’s Degree or certification as an LPN.
  • Licensed Practical Nurse in the State of Michigan.
  • BLS/CPR certification required.
  • One year of experience as an LPN with ambulatory care experience preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.

Responsibilities

  • Perform all delegated duties within the scope of education and validated competencies.
  • Assist with patient flow and clinical support functions, including scheduling appointments and managing external referrals.
  • Take and record vital signs.
  • Prepare for and assist providers with examinations and procedures, and carry out necessary clean-up.
  • Obtain lab specimens, perform tests, and document results.
  • Order and schedule lab tests.
  • Administer medications per Agency guidelines.
  • Complete clinical documentation per established Agency protocols.
  • Participate in coordination of care, which includes clinical data extraction and documentation at the direction of the RN or provider.
  • Implement provider orders using established Agency protocols when applicable.
  • Complete chart preparation for each patient, including review of the population health management system, review and update of incomplete orders and referrals, and gathering other required information (e.g., Michigan Automated Prescription Services - MAPS).
  • Record information in the electronic health record (EHR).
  • Take phone calls, gather information, and provide direction to appropriate staff.
  • Conduct follow-up calls in a timely manner, providing education and instruction within the scope of practice.
  • Participate in Continuous Quality Improvement (CQI) activities as assigned.
  • Understand and adhere to Agency compliance standards as outlined in the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy.
  • Keep up to date with all relevant federal, state, and Agency regulations, laws, and policies, including any changes or modifications.
  • Other duties as assigned.
