About The Position

The Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in long term care, under the supervision of a registered nurse, advanced practice provider, or physician. The position involves demonstrating knowledge of nursing principles for the resident in the relevant clinical setting, ensuring the health, comfort, and safety of residents, and contributing to the assessment of residents. The LPN administers medications or treatments as ordered, provides technical support to healthcare professionals, and documents resident interactions as needed. The role requires effective communication and the ability to work as part of an interdisciplinary care team.

Requirements

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Associate degree in practical nursing is preferred.
  • Must have attended a pre-licensure nursing program that included supervised clinical experience.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board.
  • Obtains and maintains required department specific competencies and certifications.
  • Completes required annual continuing education units (CEU) and competency training.

Nice To Haves

  • Experience in long term care settings.
  • IV therapy/management skills if required by the position.

Responsibilities

  • Demonstrates knowledge of nursing principles for the resident in the relevant clinical setting.
  • Ensures the health, comfort, and safety of residents.
  • Contributes to the assessment of residents and administers medications or treatments as ordered.
  • Documents resident interactions and outcomes related to resident care.
  • Skilled in establishing and maintaining effective working relationships with residents and healthcare providers.
  • Counsels residents and family members on prevention and treatment plans.
  • Performs other related duties as assigned.
