About The Position

The Licensed Practical Nurse (LPN) at Oswego Health is responsible for administering nursing care to patients under the supervision of a registered nurse. This role involves direct and indirect patient care duties, effective interaction with patients and healthcare team members, and maintaining professional nursing standards. The LPN will work in a mental health clinic setting, providing care to patients of all ages and ensuring their safety and well-being.

Requirements

  • Graduate of an approved LPN program required.
  • Current NYS licensure as Licensed Practical Nurse required.
  • Proficiency in medication administration and practiced phlebotomy skills.
  • Ability to work effectively and communicate in a team environment.
  • Excellent verbal and written communication skills, as well as active listening skills.
  • Professionalism and business acumen appropriate for working with guests, physicians, employees, and volunteers.
  • Ability to cost-effectively manage resources and supplies.

Nice To Haves

  • Current certification in CPR preferred.
  • Completion of Psychopharmacology and Emergency IV courses preferred.
  • 6 Months to 1 year of experience in a similar hospital setting preferred.

Responsibilities

  • Supervise the care of patients with direct accountability to RN charge nurse or supervisor.
  • Perform patient counseling and 1:1 observation, physical assessments, and EKG's as needed.
  • Process admissions to the In-Patient unit per policy.
  • Escort clients to appointments and for transfers.
  • Receive medications at Pharmacy and perform medication teaching under RN supervision.
  • Draw blood for ordered tests per policy.
  • Assist in planning, providing, and evaluating patient care as a member of the healthcare team under RN supervision.
  • Observe patients for reactions to drugs, treatments, and report any unusual conditions to the charge nurse.
  • Administer medication in accordance with policies and document appropriately.
  • Monitor and document client behavior and interactions, reporting any changes to the charge nurse.
  • Resolve unit, staff, and patient concerns using the appropriate chain of command.
  • Record all patient care provided in a precise and detailed manner following established procedures for charting.
  • Regularly check equipment and emergency crash cart per unit-specific policy.
  • Maintain proper level of stock and emergency supplies as required by the unit.
  • Assist patients in ambulation and transport patients as needed.
  • Demonstrate a concern for patient safety and utilize proper techniques of body mechanics.
  • Follow principles of asepsis and infection control guidelines when performing patient care.
  • Promptly answer patient call lights and identify patients by name and ID band for all direct care.
  • Provide and receive report per department-specific policy.
  • Handle telephone information with courtesy and respect for patient confidentiality.
