We are hiring a Private Duty LPN in the Bulls Gap, TN area to work from 6a-6p M-F. Case Specific Enhanced Pay Rates are available. We offer flexible scheduling and weekly pay! Additionally, there is a $750 bonus after your first 300 worked hours. Requirements

  • Must have current LPN licensure in the state of Tennessee.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification required.
  • Previous home care experience preferred.

Responsibilities

  • Provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes.
  • Record clinical and progress notes and coordinate other patient care activities/services.
  • Provide high quality clinical services within the scope of practice as defined by state laws.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Participate in the agency's care coordination process including timely reporting of patients' needs and changes in condition.
  • Attend patient case conferences and communicate effectively with patients, families, physicians, and other clinicians.
  • Ensure confidentiality of patient health information per agency policy.
