Wages: LPN Wage: $22.50-$26.50/hr, based on experience Shift and weekend differentials available

Requirements

CPR certification
AED response training
Must have active Licensed Practical Nurse (LPN) license

Responsibilities

For Residents living in their independent living home, perform clinical assessments, healthcare treatments, wellness checks, and respond to and provide urgent healthcare needs when the call system is activated.
Per physician orders and existing policies, conduct assessments, perform diagnostic testing, complete treatments, administer medications and immunizations, and perform wound care.
Liaison between Residents, their family, and their primary healthcare providers.
Provide assistance to create and implement a plan of care based on Resident changes in health status facilitating transitions of care throughout the continuum.
Team Member onboarding: administer and read tuberculin skin tests, give immunizations, assist with assessment of work-related injuries.
Provide Resident education.
