About The Position

Tabor Hills Senior Living Campus, a not-for-profit community located in Naperville, IL, has been dedicated to caring for our senior population for over 125 years. Tabor Hills Senior Living offers an Independent Living Community, Supportive Living Community and a Healthcare Facility (skilled nursing care, post-acute medical and rehabilitation, respiratory therapy.) We have a rare opportunity for a part time Staff Nurse to join our Supportive Living Campus! Under the supervision of the Director of Healthcare Services, this position is responsible for conducting assigned health assessments, developing service plans, performing health monitoring and personal care programs of the community. In addition, this position will assist the Director as necessary and will be in charge in the absence of the Director.

Requirements

  • Valid Illinois RN license.
  • Current CPR Certification.
  • At least one (1) year of experience in a geriatric care environment.
  • Must be able to stand for extended periods of time.
  • Mobility essential, walking, standing, sitting, talking, hearing.
  • Work in a safety conscious manner, which ensures that safe work practices are used in order not to pose a risk to others in the workplace
  • Must be energetic, upbeat, patient, and empathetic
  • Complies with company policies and procedures and local, state, and federal regulations
  • Maintains a dependable attendance record and adheres to standards of cleanliness, grooming, hygiene, and dress code
  • Proof of completion of Covid -19 Vaccination Series required.

Responsibilities

  • When the resident's health condition warrants, inform Director of Healthcare Services and perform assessment with appropriate direction from Director of Healthcare Services.
  • Develops assigned service plans for residents based upon the resident assessment(s) under the direction of Director of Healthcare Services. The service plan should be designed to meet the personal care needs of the resident and to monitor significant health conditions, wi1ile also promoting resident independence and dignity.
  • Communicates with resident's physician(s), other pertinent health care providers and family as to the ongoing health status of the residents.
  • Refers residents to appropriate medical care when needed. Conducts follow-up care as needed.
  • Assigns the various personal care tasks contained in the resident's service plans to the Certified Resident Attendant's on an ongoing basis.
  • Assists, as assigned, in the community's resident medication monitoring and set-up program.
  • Provides health promotion services, disease prevention counseling, and teaching self-care to residents on an assigned basis.
  • Provides, as assigned, general updates and progress notes to resident's records.
  • Completes all other duties as assigned by Administration.
