About The Position

At NCC, the LPN works under the supervision of a RN and is primarily responsible for safe, accurate medication administration and documentation within the Community Support Services Program. This detail-oriented and empathetic professional must demonstrate strong critical thinking and problem-solving skills, ensure precise transcribing of physician orders, maintain thorough and compliant documentation, and support effective medication management and administrative processes.

Requirements

  • Active Licensed Practical Nurse (LPN) license in Maryland and the District of Columbia
  • Valid driver's license and own vehicle (mileage will be reimbursed)
  • Diploma/certificate from a two-year college or technical school in Nursing
  • CPR/ First Aid certification
  • Must be licensed in the District of Columbia and Maryland as an LPN
  • COVID-19: You are required to be fully vaccinated for COVID-19 prior to your first day of employment.

Responsibilities

  • Administers medications to multiple site locations, and day program
  • Performing nursing assessments to people supported as needed
  • Monitoring patients’ overall health during home visits.
  • Monitors and update current MAR’s with physician orders and review MAR’s for un-signed medications each month and RN will co-sign.
  • Review daily medication refills orders and take to the home as needed
  • Review and distribute monthly MAR’s and medications
  • Schedule appointments, and prepare specialty clinic consults as recommended by M.D.
  • Provides nursing coverage for the day program as needed.
  • Attends Interdisciplinary Team Meetings as needed.
