Requirements

  • Active Missouri or Compact LPN License
  • Minimum 1 year of LPN experience (Home Health preferred)
  • Valid driver's license & current auto insurance
  • Current CPR certification
  • Ability to lift 50 lbs
  • Strong communication, organization, and self-management skills

Responsibilities

  • Assist in developing, reviewing, and updating patient plans of care
  • Deliver nursing care under the physician's plan of treatment and client needs
  • Observe, document, and communicate changes in patient condition
  • Educate clients, families, and care team members
  • Participate in ongoing training, nurse meetings, and quality initiatives
  • Support the DON with patient assignments and referral oversight
  • Begin discharge planning upon admission and complete discharge summaries
  • Conduct HHA supervisory visits
  • Complete PRN visits; participate in rotating weekend and on-call coverage
  • Assist with quality improvement, chart audits, and problem identification
  • Communicate daily with the DON regarding patient status and concerns
  • Travel locally to client homes and outreach locations
  • Ensure compliance with all Phoenix policies and federal/state regulations
  • Perform additional duties as requested
