Position Overview: The Licensed Practical Nurse (LPN) will work under the supervision of the Clinical Service Director. The LPN is responsible for: coordinating, monitoring, and providing health care services and treatment for Bluestem PACE participants; and coordinating quality improvement development, support, data collection and analysis.

Requirements

  • Must be at least 18 years of age.
  • Must be a Licensed Nurse registered to practice in Kansas.
  • Must be certified in Cardio Pulmonary Resuscitation (CPR).
  • Must possess working knowledge of Federal and State regulations.
  • Must possess proficient assessment skills related to geriatric participants.

Nice To Haves

  • Prefer one year of experience working with the frail or elderly; absent such experience, will upon hire, receive from Bluestem PACE appropriate training in working with the frail or elderly population.
  • Prefer clinical background with three to five years of experience in geriatrics.

Responsibilities

  • Facilitate the integration of new participants into the Bluestem PACE health care delivery system including medication, immunizations, and routine monitoring of chronic problems through the health record and nursing care plan development.
  • Provide a high level of customer service to the participants, families and other employees.
  • Submit PACE data requirements for caseload participants.
  • Follow established standards of nursing practices and implement program policies and procedures.
  • Monitor and deliver routine nursing care; document progress notes and nursing care plans on a regular basis.
  • Provide liaison with primary care provider in the event of an episodic illness; assist in coordinating services provided by primary care provider.
  • Administer and monitor participants’ medications prescribed by the physician.
  • Assist participants in maintaining optimum health; provide health education and counseling to participants and caregivers facing chronic conditions and end of life issues.
  • Assure nursing services are documented in a timely, legible and accurate manner.
  • Document care in the participant’s medical record consistent with documentation policies and workflows.
  • Prepare and administer medications, enteral nutrition, parenteral nutrition and treatment as ordered.
  • Report changes in the participants’ conditions to a physician or responsible party promptly and take follow-up action as necessary.
  • Follow Bluestem PACE admission, discharge, and transfer procedures and coordinate admissions, discharges and transfers with other departments, physicians, and ancillary service providers.
  • Actively participate in Case Management.
  • Work directly with the members of the interdisciplinary team; participate in the development of the plan of care.
  • Provide updates to team regarding participants’ change in health or functional status.
  • Attend and participate in staff meetings, in-services, projects and committees as assigned.
  • Follow and evaluate the implementation of the Participant’s Bill of Rights.
  • Assume responsibility for compliance with federal, state and local regulations.
  • Contribute input to monthly quality assurance data.
  • Undertake responsibility for participant safety; identify safety hazards and emergency situations and initiate corrective action as able.
  • Provide basic first aid to agency employees as required by incident or injury.
  • Assist in assuring personal and staff comply with infection control, personal protective equipment, blood borne pathogens, hazardous material handling and fire safety.
  • Adhere to and support the facilities’ policies, practices and procedures.
  • Accept assigned duties in a cooperative manner and perform all other related duties as assigned.
  • Must be flexible in schedule of hours worked and have ability to be on-call.
  • Understand that use of personal vehicle may be required at times.
  • Coordinate clinic schedule and primary care provider’s schedule.
  • Facilitate the work flow of the clinic related to urgent and unscheduled clinic visits.
