Through our Center for Connected Care, we provide a full continuum of care so that patients can recover from illnesses or injuries in the very best location for their individual needs. This is a hybrid position with on-site days located at Independence, OH. A caregiver in this position works Monday through Friday from 9:00am to 5:30pm. Requirements include every 5th weekend from 8:00am to 4:30pm and 2 holidays per year from 8:00am to 4:30pm. A caregiver who excels in this role will: Facilitate the timely and appropriate initiation of home health and infusion pharmacy services for all referred patients. Coordinate with CC's home care agency as well as outside partner agencies to confirm timely delivery of pharmacy supplies and medication and timely initiation of homecare services. Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse. Clinical review for first dose, timing and safety of requested pharmacy service ensuring appropriate medical device is available for therapy. Coordinate initial and final orders on internal and external referrals for service under CC home care and outside agencies for patients needing home care nursing, therapy, social work and pharmacy services. Ensure accurate and timely pharmacy orders are obtained. Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate. Be responsible for confirmation of home care and pharmacy benefits. Communicate the home care and/or infusion pharmacy needs with scheduling, managers, reimbursement, and other parties involved in the home care services process. Serve as the CCHCS information specialist. Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines. Requires manual dexterity to perform work on a PC. Must be able to carry / transport office equipment including a desktop, laptop and monitor to home / work environment.
