As an LPN Central Admissions, you will facilitate the timely and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for all referred patients. A caregiver in this position works days from 9:00AM - 5:30PM. This position is remote but requires hybrid training on site in Independence, OH.

Requirements

• Graduate from an accredited Practical Nursing Program.
• Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).

Nice To Haves

• One year of clinical experience in Homecare.

Responsibilities

• Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered Nurse.
• Coordinate initial and final orders on internal and external referrals for patients needing home care nursing, therapy and social work services.
• Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches when appropriate.
• Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
• Serve as the CCHCS information specialist.
• Understand other services and products available through CCHCS, including criteria for service and reimbursement for all the home care business lines.
