About The Position

The LPN Care Coordinator is responsible for developing and providing care coordination services to patients enrolled in Alopex programs. This role supports patients across the continuum of care by collaborating with providers, specialists, and care teams to ensure timely, high-quality, and patient-centered care. The ideal candidate has skilled nursing experience, strong care management skills, and is fluent in Spanish.

Requirements

  • Active LPN license in good standing.
  • Bilingual (English/Spanish) – Required.
  • Minimum of 2 years of patient-centered care experience.
  • Minimum of 1 year of skilled nursing experience.
  • Experience working with electronic medical records (EMR).
  • Ability to perform nursing assessments, problem identification, and care plan development.
  • Strong computer skills and adaptability to new technology.
  • Excellent time management and organizational skills.
  • Strong critical thinking, assessment, and problem-solving abilities.
  • Clear verbal and written communication skills.
  • High attention to detail with a commitment to accurate documentation.
  • Dependable, goal-oriented, and patient-focused.

Responsibilities

  • Conduct detailed reviews of electronic medical records (EMR).
  • Develop, implement, and maintain individualized care plans.
  • Coordinate all care management activities across the continuum of care.
  • Communicate effectively with patients, primary care providers, and interdisciplinary care teams.
  • Perform timely care management and follow-up activities.
  • Monitor and evaluate patient progress toward established outcomes.
  • Maintain accurate and thorough documentation related to medications, diagnoses, goals, interventions, preventive care, and outcomes.
  • Conduct monthly care coordination calls with enrolled patients.
  • Address patient needs and concerns, including follow-up actions as required.
  • Educate patients on chronic conditions and reinforce provider-directed treatment plans.
  • Assess social determinants of health and connect patients with appropriate community resources; document accordingly.
  • Identify and respond to changes in patient conditions in a clinically appropriate manner.
  • Adhere to all Medicare, Shifox, and Alopex policies and procedures.
  • Perform other duties as assigned.
