About The Position

At Home Harmony is seeking a highly organized, dependable, and personable Care Concierge LPN to join our growing home-based medical and pharmacy practice serving aging adults. This role is based in a small clinic within an upscale senior living community in Mechanicsville, VA . The Care Concierge LPN plays a vital role in supporting patient care, managing clinic operations, and delivering exceptional customer service. This position requires a collaborative team player who can multitask effectively, work independently, and approach all responsibilities with professionalism, compassion, and respect for elderly patients.

Requirements

  • Licensed Practical Nurse (LPN) required
  • Strong computer proficiency with EMR systems

Nice To Haves

  • Family medicine and/or geriatric experience preferred
  • Athena EMR experience preferred

Responsibilities

  • Facilitate efficient patient flow with patience, respect, and attentiveness to the needs of elderly patients
  • Triage and room patients
  • Measure, document, and record patient vital signs in the EMR
  • Perform phlebotomy and process in-house laboratory specimens
  • Follow established procedures for ordering, processing, and tracking laboratory specimens and diagnostic tests
  • Prepare, clean, and stock examination rooms
  • Return patient phone calls and provide consistent, timely communication regarding clinical questions
  • Coordinate medication delivery and administer injections as appropriate
  • Obtain prior authorizations and referrals for medications, treatments, and procedures
  • Adhere to all clinical, safety, and performance standards and protocols
  • Provide care with compassion, empathy, and cultural competency
  • Maintain positive, professional relationships with At Home Harmony staff
  • Attend training sessions and clinical team meetings as requested
  • Serve as an ambassador for At Home Harmony by maintaining a professional demeanor at all times
  • Provide case management services under Medicare programs including Chronic Care Management (CCM) and Remote Patient Monitoring (RPM)
  • Support primarily Senior Living Community patients, with potential to assist patients throughout the Greater Richmond community
  • Perform care management activities including phone-based patient engagement, EMR documentation, and use of additional clinical systems
  • Periodically travel to senior living communities or patient service locations, including time spent riding in a vehicle
  • Develop comprehensive, individualized care plans for enrolled patients
  • Assist patients through transitions of care, including hospital discharges, rehabilitation admissions/discharges, follow-up appointment scheduling, home health coordination, and other patient-specific needs
  • Conduct in-person assessments as needed to initiate and set up RPM devices
