About The Position

As a Care Coach at CircleLink Health, you will play a crucial role in managing patients with chronic conditions enrolled in Medicare's Chronic Care Management program. This remote position requires you to work 20-25 hours per week, focusing on educating patients about self-management skills and goal setting, while implementing and improving their Plan of Care. You will utilize motivational interviewing techniques to assist patients in managing their health and reducing care gaps, ultimately contributing to the mission of shifting towards preventative care.

Requirements

• Current, unrestricted Compact LPN license
• 5+ years experience as a Licensed Practical Nurse
• Proficiency with electronic health records and web-based applications
• Strong communication and telephonic skills
• Excellent organizational and time management skills
• Detail-oriented and self-directed with the ability to work independently.

Nice To Haves

• Case Management or Chronic Disease Management experience
• Case Management Certification
• Certified Diabetes Educator
• Transitional Care Management experience
• Experience with Motivational Interviewing or other behavior change communication techniques

Responsibilities

• Educate patients on self-management skills and goal setting for chronic conditions.
• Implement and improve the Plan of Care by updating medications and appointments, recording biometrics and vital signs, and providing care coaching.
• Utilize Motivational Interviewing or other behavior change techniques to assist patients with self-management.
• Conduct Transitional Care Management activities for high-risk patients discharged from the hospital to reduce readmissions.
• Encourage preventive care and chronic care management, including annual well visits and medication management.
• Connect patients with community resources as needed.

Benefits

• 1099 contract position with no end date
• Compensation of $10.00 per initial clinical encounter per patient per month
• Additional compensation for patient withdrawals and unsuccessful contact attempts
