About The Position

Are you a new or seasoned LPN with a passion for behavioral health looking for a PT role? At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. As a Licensed Practical Nurse you’ll play a critical role in carrying out that mission by supporting adults with Intellectual and Developmental Disabilities at our warm, home-like Adult Crisis Therapeutic Home located in Woodbridge, VA. Position details: Part-time, Friday and Saturday 11p-11a Location: Woodbridge, VA

Requirements

Licensure: Current valid, unrestricted licensed as a Practical Nurse in state of Virginia
Knowledge: MH and I/DD systems of care preferred
Skills: Strong communication and documentation
Experience: Crisis prevention and intervention services preferred
Transportation: A valid driver's license and car insurance for occasional transportation to appointments

Responsibilities

Provide compassionate medical and nursing support to individuals with intellectual/developmental disabilities and co-occurring behavioral health needs
Administer medications safely and accurately in accordance with clinical protocols
Conduct nursing assessments
Educate and empower individuals regarding their mental and physical health
Collaborate closely with interdisciplinary team members to ensure coordinated, person-centered care
Maintain thorough and timely documentation of all nursing services and observations
Uphold compliance with all regulatory and clinical care standards to ensure the highest quality of service
