About The Position

RPH is looking for LPN candidates who are passionate about caring for elders. Responsibilities include but are not limited to; Medication monitoring and administration, acting as clinical lead to staff, case management, care planning and oversight, personal care assistance as needed, meal planning and enrichment recreation as needed. Requirements

  • 1 year of elder care or direct care experience is preferred but not required.
  • NYS Nursing Licensure (LPN) required.
  • All candidates must be willing to work rotating weekends and rotating holidays

Responsibilities

  • Implement and evaluate the delivery of health services to elders with memory impairment.
  • Supervise, provide assistance and oversite with medication management and administer routine and prn medications including diabetic management, insulin injections and blood glucose tests in compliance with nursing standards and NYS regulations.
  • Ensure progress notes are informative and descriptive of services provided and of the elders’ response to services.
  • Assist in set-up of medical record upon new admission; medication list, MAR, admission note, service delivery record etc.
  • Assist in providing daily care to Elders.
  • Function as an advocate for elders, coordinating and linking all aspects of their care with the case manager.
  • Ensure that Care Partners/Shahbaz are aware of each elder’s wellness plan and that these plans are used to provide daily care.
  • Make daily rounds to ensure household roles are completed. Assist as needed.
  • Investigate complaints and grievances made by elders or families and communicate to leadership.
  • Develop and implement effective communication systems between staff; foster positive interpersonal relationships.
