About The Position

Job Objective: Coordinates delivery of nursing and social services for adult day participants

Requirements

• Graduate of accredited nursing school
• 2-3 years of nursing experience.
• State of Connecticut Nursing License
• CPR certification
• Demonstrates clear, compassionate, and effective nursing, demonstrating excellent judgement.
• Communicates effectively with clients, employees, and care partners.
• Ability to maintain positive working relationships both within the organization and community.
• Empathetic, culturally aware, inclusive, and person-centered of all we serve.
• Respect client privacy and dignity.
• Creative, supportive, and educational approach to working with clients with a variety of needs and reactions to new life circumstances.
• Ability to juggle multiple tasks and accomplish deadlines in an organized fashion.
• Utilizes Microsoft suite, including outlook, and excel; flexibility and initiative to adapt to changing software technology.
• Ability to travel to client homes and other community sites as necessary related to networking and intakes.
• Availability of personal vehicle for work related travel (mileage reimbursed). Travels primarily within Fairfield and New Haven counties.
• All other related duties as assigned.

Nice To Haves

• Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

Responsibilities

• Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.
• Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
