As our ACT nurse, you will work as a member of the Assertive Community Treatment (ACT) Team, serving as a critical component of care to adults with severe mental illness in their homes, places of employment, and communities. You will work with a team of multidisciplinary staff to help each client succeed in being able to live happy, successful, and productive lives, all while being able to stay integrated with the friends, family, and community. In your role, you will meet with clients in the home, community, and office to provide necessary medical treatment and support services such as basic vitals and labs, facilitating administration of long acting injectables, medication management support, and coordinating medical provider care. This position includes client transportation using a company car, as needed and a one week on-call rotation every 7-10 weeks. On-call is handled virtually or in person, depending on the situation, and earns additional PTO. For optimal client success, you will meet with clients in the local community primarily, with some time spent in the office or via zoom/phone. Assertive Community Treatment (ACT) was created in the 1970s and is one of the oldest and most widely researched evidence-based practices working with severe mental illness adult clients. By providing person centered assistance including housing, recovery, vocational support, nurse/psychiatrist and therapist services, we enable clients to eventually transition to lower levels of care programs or move toward independence, increasing their quality of life.
