LPC/LCSW
About The Position
Licensed Professional Counselor (LPC)/Licensed Clinical Social Worker (LCSW) will provide Behavioral Health services to all assigned patients. LPC/LCSW will work closely with the Behavioral Health Director and Behavioral Health Coordinator to ensure best practice techniques are utilized when addressing the needs of patients. LPC/LCSW will assess and monitor patents' progress by adhering to agreed upon treatment plan. LPC/LCSW will maintain efficient record keeping and clinical documentation via EMR System. LPC/LCSW will participate in all clinical staff meetings by presenting any problematic case that needs group consultation.
Requirements
- LPC/LCSW with a master's degree from an accredited university
- A minimum of three (3) years of job-related experience, preferably in a Community Health Care Clinic environment with the responsibility of providing behavior health counseling services.
- Ability to read, write, speak, and comprehend written documents fluently.
- In good standing with Louisiana State Licensure Board.
- Proof of professional certification, licenses or registration required.
- All candidates will be required to submit to a criminal background check and screening as part of pre-employment.
Responsibilities

- assessing patients using standardized assessment/screening tools or measures.
- provide brief, solution focused therapy.
- provide group psychoeducation/treatment related to chronic medical conditions.
- provide information and referral services.
- support care coordination.
- perform crisis assessments/triage with appropriate interventions.
- provision of other services related to wellness.
- ensure proper documentation of services provided in accordance with requirements in the electronic health record.
- provide other related activities designed to provide integrated health care services.
- provide individual psychotherapy with children, adults, and families.
- assist and support scheduling in electronic health record.
- provide psychotherapy to children and adults.
