About The Position

The position involves developing, assessing, and coordinating holistic care management activities primarily focused on populations with significant mental and behavioral health needs. The role aims to enable quality, cost-effective healthcare outcomes by evaluating member service needs, developing care plans, and acting as a liaison between members, families, and healthcare providers.

Requirements

  • Master's degree in Mental Health or Social Work or Graduate from an Accredited School of Nursing.
  • 2 - 4 years of related experience in mental health or social work.
  • Licensed Behavioral Health Professional or RN with psychiatric experience (LCSW, LPC, or RN with BH experience).

Nice To Haves

  • Master's level unrestricted licensed behavioral health clinician; LCSW, LPC, or Psych/Mental Health RN.
  • 3+ years of case management experience with adults with serious mental illness.
  • Experience in patient advocacy or coordinating community mental health services.
  • Experience in in-patient/out-patient facilities or community-based Behavioral Health settings.
  • Experience administering psychological assessments and managing high volume caseloads.
  • Bilingual in Spanish and English.

Responsibilities

  • Develop and coordinate holistic care management activities for members with mental/behavioral health needs.
  • Evaluate member service needs and contribute to care/service plans.
  • Act as a liaison and advocate for members between families, physicians, and facilities/agencies.
  • Perform frequent home and/or site visits to assess member needs and collaborate with resources.
  • Educate long-term care members and their families on preventive care and healthcare benefits.
  • Provide operational support for mental and behavioral health activities, including crisis calls and medication monitoring.
  • Coordinate community resources, including medical, behavioral, and social services.
  • Ensure appropriate referrals based on individual member needs and support the identification of providers and specialists.
  • Maintain accurate documentation in the electronic care management system.
  • Provide feedback to leadership on opportunities to improve care quality and service delivery.
