About The Position

This position manages and is responsible for both the Title XIX (Medicaid) Case Management and Care Coordination units, including core operations of the Health Home Lead program. This includes compliance with applicable federal and state regulations, development of staff training, participation in state workgroups/committees, and building a positive culture within the unit. This position is a member of the agency’s Extended Management Group.

Requirements

  • Bachelor’s Degree in a relevant field AND Two years of experience providing direct human services AND two years of supervisory experience OR Master’s degree in a relevant field AND One year of experience providing direct human services AND two years of supervisory experience
  • Possession of a valid driver’s license and minimum state-required vehicle insurance and have use of reliable transportation.
  • Good working knowledge of supervising direct reports that fosters an environment of learning, growth and success
  • Good understanding and the ability to apply internal policies and procedures, as well as federal, state, and local employment laws to the supervisory process.
  • Ability to establish and maintain effective working relationships.
  • Ability to collect, analyze, and evaluate data to manage program effectiveness.
  • Ability to evaluate pending legislation and its impact to programs.
  • Ability to develop and implement work plans for program and system improvement.
  • Ability to manage and complete multiple work projects with quality results in a timely manner.
  • Ability to develop, administer, and participate in quality program reviews and to develop work plans for improved outcomes.
  • Knowledge of Washington State Medicaid programs, Medicaid Long-Term Services and Supports, and Case Management Services.
  • Ability to lead, facilitate or participate in meetings and work groups to achieve desired results.
  • Ability to make presentations and provide training one-on-one and in groups.
  • Ability to travel to and from client’s homes, meetings and community events which might not be ADA accessible.
  • Ability to communicate verbally in the English language in face-to-face one-on-one settings, in group settings, by personal computer, or using a telephone.
  • Ability to produce written documents with clearly organized thoughts using proper English sentence construction, punctuation, and grammar.
  • Computer and software skills, ability to use Microsoft Office applications, the Internet and SSPS and other ADSA applications as management tools; ability to operate general office equipment; work at a desk using phone and computer for up to eight hours a day.

Responsibilities

  • Provides leadership, direction, and coaching to Care Coordination, and Title XIX Case Management Supervisors.
  • Ensures programmatic and operational compliance with federal Medicaid regulations, DSHS – Home & Community Living Administration Management Bulletins, Long-Term Care Manual, Health Care Authority Health Home policies, and Agency policies and procedures.
  • Collaborates with Quality Assurance/Training Coordinator and HCA to identify and develop training.
  • Completes quality assurance desk reviews for AAADSW-contracted CCO network and communicates trends and annual DSNP Monitoring in compliance with the DSNP Monitoring Rationale.
  • Prepares and maintains required reports, training registrations and records, gathers, and refers relevant information to the LTSS Director.
  • Develops and sustains a team-based work environment.
  • Works collaboratively with key stakeholders inside and outside the agency. This includes DSHS Home & Community Living Administration, Health Care Authority, other AAAs, community-based organizations, and healthcare providers.
  • Conducts annual Performance Evaluation of direct reports.
  • Participate in the full employment lifecycle for direct reports, including hiring and onboarding, developing, coaching, and monitoring of employee performance and performance improvement plans, progressive discipline, and offboarding.
  • Works effectively with a wide range of diverse staff, clients, families.
  • Manages Health Home enrollment, disenrollment, eligibility, and referral information with HCA and contracted Medicare Advantage (MA) Plans.
  • Facilitates the Health Home Basic Training annually in compliance with HCA standards.
  • Participates in essential activities in alignment with HCA Training.
  • Coordinates with the Contracts team to facilitate monitoring, review and distribution of CCO network contracts, and communications.
  • Coordinates with fiscal support personnel to ensure accurate billing and manages billing discrepancies.
  • Coordinates with contracted service providers to manage network operations for the HAP Database, Insignia Flourish, PreManage/Point Click Care, and PRISM.
