About The Position

We are actively seeking a Long Term Care Social Worker You will join our amazing team in one of our Long Term Care Units. We are excited to launch a comprehensive program that will also include therapeutic activities and social services coordination on the unit. Come join us and lend your enthusiasm and ideas to this new program, aimed to meet the psycho-social needs of our residents through dynamic activities and compassionate social services. Schedule: Full time 40 hours a week

Requirements

Degree in Social work or related field of study
2 years' experience in Human Services or applicable field
Passion for the elderly
Ability to engage with families, residents and team
Excellent follow through and communication skills
Ability to manage moving priorities each day
Strong verbal and written communication skills
Computer skills for charting and documentation
Strong team player with the ability to have FUN!

Nice To Haves

Prior experience in activities is a plus

Responsibilities

Comprehensive psychosocial assessments
Developing and maintaining a plan of care in collaboration with the team
Coordinating dynamic activity programs and documentation on the unit
Resource referral and contact with families of residents to provide support and/or collaboration towards goals of the family and resident
Crisis and therapeutic interventions
Perform assigned resident interviews that assess each individual mood, behavior, and cognitive ability and write the accompanying summary of findings
Provide education and techniques to unit staff of person-centered interventions that can be utilized to address any mood, behavioral, or cognitive concerns residents might present with.
Meets deadlines for time sensitive responsibilities
Ability to work with charge nurse to lead the Unit team
