About The Position

The Long-Term Care Registered Nurse (RN) delivers comprehensive nursing care to residents across three long-term care units. The RN is responsible for providing individualized, age-specific care that focuses on enhancing quality of life, promoting comfort, and supporting residents' health needs throughout their stay. This role requires strong clinical skills, compassionate care delivery, and effective communication to meet the unique needs of long-term care residents and their families in a supportive and patient-centered environment.

Requirements

  • BLS certification accredited by the American Heart Association (AHA)
  • Unrestricted license to practice nursing in one of the States, Territories, or Commonwealths of the United States or the District of Columbia
  • Minimum of three (3) years of current nursing experience, including experience within a long-term care unit.
  • ASN accepted; BSN preferred
  • Education must have been completed at a school accredited by the ACEN or CCNE.

Responsibilities

  • Assesses, plans, implements, and evaluates care based on age-specific components
  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care
  • Provides family education
  • Takes responsibility for patient ADLs (activities of daily living), assessments, BCMA medication and procedures per established policies and guidelines (i.e.)
  • Barcode medication and administration, IV therapy, blood transfusion, monitor blood pressure, weight and hydration
  • Administration of tube feeding
  • Management of behavioral patients
  • Participate in interdisciplinary team participation
  • Complete admissions, discharges, and internal and external transfers
  • Delegate to ancillary staff
  • Completion of electronic medical record documentation
  • Patient transport
  • Emergency response procedures.
