About The Position

Long-term care planning can be overwhelming for individuals and families navigating aging-related challenges. The Long-Term Care Planning Assistant plays a vital role in supporting older adults and their loved ones by helping coordinate care-focused planning, gathering critical information, and ensuring each client experience is handled with compassion, clarity, and professionalism. Requirements

  • 2-3 years of experience in caregiving, social work, long-term care, or related fields.
  • Strong organizational and communication skills.
  • High level of empathy and professionalism.
  • Ability to work independently and collaboratively.
  • Proficiency with Microsoft Office.

Nice To Haves

  • Bachelor's degree preferred or equivalent experience.

Responsibilities

  • Support Long-Term Care Planners and attorneys with all aspects of long-term care and Medicaid-related planning.
  • Compile, organize, and maintain detailed client information.
  • Assist in preparing and tracking long-term care and Medicaid documentation.
  • Attend client meetings with planners and attorneys.
  • Communicate with long-term care facilities and caseworkers.
  • Serve as a witness and/or notary when needed.
  • Maintain electronic and hard-copy files.
  • Prepare and proofread correspondence.
  • Answer and route phone calls appropriately.
  • Perform general office and administrative tasks.
  • Maintain strict confidentiality.
  • Perform other duties as assigned.
