Patient Engagement Advisors (PEA) is seeking a full-time Logistics Transition Specialist to work onsite at AdventHealth Tampa and in the local area while performing Logistics Courier duties. The position is scheduled from Monday to Friday, 9:30 am to 6:00 pm, and the PEA will provide a vehicle for the use of logistic courier duties only. The Logistics Transition Specialist plays a vital role in achieving the vision and mission of PEA. To accomplish these goals, the Transition Specialist must exhibit empathy, innovation, collaboration, teamwork, passion, dedication to personal development, and a strong commitment to 'know the patient'. This role is responsible for encouraging patients to actively participate in their care experience during and after their hospital stay. The Transition Specialist will ensure that patients are provided with services, educated on their discharge requirements (including medications, follow-up care, nutrition, transportation, and assistance programs), and equipped with the necessary resources for success in their next stage of care. The Logistics Transition Specialist is tasked with delivering medications, nutrition, product kits, and other patient care-related items to homes or other hospital locations for handoff. This position requires access to Protected Health Information (PHI), and team members are expected to comply with all confidentiality and security policies related to the use, access, and disclosure of PHI. The essential responsibilities include face-to-face and telephonic patient engagement, delivery of care items, documentation of social determinants of health, utilization of applications for patient needs, and participation in hospital-specific rounds to contribute to the clinical care team. The role also involves ensuring compliance with payment collection policies, facilitating available services, and maintaining a high level of professionalism while reflecting the PEA brand.
