Requirements
- Licensure: Must hold a current Doctor's license from the State of Texas Medical Board.
- Residency: Successful completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Certifications: Current Drug Enforcement Agency (DEA) registration certificate.
- National Provider Identifier (NPI#)
