Requirements

  • Associate's degree in nursing, BSN preferred
  • Must possess a registered nurse license
  • Two (2) years of experience working in a clinic setting
  • Excellent verbal and written communication skills
  • Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Computer Skills To perform this job successfully, an individual should have thorough knowledge in computer information systems.

Responsibilities

  • Receive and Manage Patient Referrals: Accept all referrals for patients who have received a new diagnosis or are experiencing a recurrence of their condition. Ensure timely processing and triaging of these referrals to facilitate prompt evaluation and care
  • Patient Assessments: Perform thorough assessments of patients in accordance with established clinical practice guidelines, ensuring all necessary diagnostic evaluations, imaging, and laboratory tests are completed accurately
  • Post-Operative Follow-Up: Reach out to newly diagnosed patients following their post-operative visits to discuss their individualized treatment plans, address any concerns, and schedule follow-up appointments to monitor their progress.
  • Weekly Pathology Report Review: Systematically review and analyze pathology reports from Transurethral Resection of Bladder Tumor (TURBT) procedures on a weekly basis to stay informed about patient outcomes and adjust treatment plans as needed
  • Oversee Care Coordination Plans: Develop, implement, and oversee comprehensive care coordination plans that facilitate seamless transitions within the healthcare system.
  • Resource Provision and Barriers Assessment: Identify and provide resources to patients while also recognizing barriers to treatment. Work to eliminate these barriers by coordinating with other services and support systems.
  • Coordinate Comprehensive Patient Care: Manage and streamline patient care, including the scheduling of appointments, transitions of care, and communication among the healthcare team. Collaborate closely with physicians and various specialists to ensure coordinated services across the continuum of care.
  • Holistic Patient Needs Assessment: Assess both current and future needs of patients, covering a broad spectrum of medical, psychosocial, and financial factors. Make appropriate referrals to both internal support services and external community resources as necessary.
  • Interface with Healthcare Teams: Actively engage with other healthcare teams to facilitate appropriate referrals and ensure patients receive relevant services tailored to their unique needs.
  • Patient and Caregiver Education: Provide, reinforce, and enhance education for patients, families, and caregivers about their diagnosis, available treatment options, side effect management strategies, and the importance of post-treatment care and survivorship.
  • Clinical Trials Promotion: Actively promote awareness and understanding of clinical trials among patients and their support networks, highlighting potential benefits and opportunities for involvement.
  • Documentation of Patient Encounters: Ensure meticulous documentation of all patient encounters, services provided, and care coordination activities in designated electronic health record systems for outpatient and inpatient settings.
  • Ongoing Treatment Assessment: Continuously evaluate patients’ treatment progress and educational needs, adapting care plans accordingly to optimize outcomes.
  • Relationship Maintenance: Maintain a strong and supportive relationship with patients and their families throughout the treatment process, fostering trust and open communication.
  • Secure Documentation Practices: Uphold the highest standards of confidentiality regarding patient information, complying with all regulations and policies related to patient privacy.
  • Travel for Business Needs: Exhibit flexibility and willingness to travel to other COUG locations to meet business needs and support patient care initiatives.
  • Perform Related Duties as Required: Undertake any additional responsibilities as needed to support the healthcare team and enhance patient care delivery.
