About The Position

The MSW Rehab Social Worker at Hope Network plays a crucial role in providing clinical services to individuals in neuro rehabilitation. This position involves evaluation, diagnosis, treatment planning, and discharge planning, ensuring a smooth transition for patients from the program to their homes or other care levels. The social worker will work as part of an interdisciplinary team, supporting families and individuals in overcoming challenges and achieving meaningful outcomes.

Requirements

  • Master's degree in Social Work (M.S.W.) from an accredited college or university required.
  • Licensed as a Social Worker by the State of Michigan (Clinical designation).
  • At least two years of experience in a health/human service field.

  • Experience in neuro rehabilitation settings.
  • Familiarity with community resources and social services.

Responsibilities

  • Provide clinical services including evaluation, diagnosis, psychosocial assessments, treatment planning, family assessment, and adjustment support.
  • Lead the interdisciplinary team in assessing the person-served and family's ability to cope with challenges and support long-term goals.
  • Provide families with information and updates on progress toward goals.
  • Identify a clear picture of the patient's pre-injury lifestyle and interests to maximize meaningful outcomes.
  • Ensure smooth transitions for individuals discharging from the program to home or other care levels.
  • Assist with discharge planning, including housing and community resource connections.
  • Secure support for discharge plans from all stakeholders.
  • Assist families with SSDI/Medicaid applications and medical benefits.
  • Serve as a primary therapist for disability adjustment and psycho-emotional services, if designated.
  • Provide individual and family counseling, substance abuse intervention, and crisis management.
  • Consult and train staff on interpersonal, social, and behavioral functioning of individuals/families.
  • Participate in developing psychosocial curriculum and family services.
  • Document activities for billing and ensure timely completion of clinical reports.
