The LMSW Consultant position at Personal Healthcare Management LLC is a vital role designed to support the Director of Social Work Services in ensuring that the psycho-social interests of residents are met. This position involves planning, organizing, implementing, evaluating, and directing the overall operation of the Social Work Services Department in accordance with current Federal, State, and Local standards governing the facility. The consultant will work remotely with one onsite visit per week at various skilled nursing facilities (SNFs) across regions including Delhi, Norwich, Albany, CNY, and North Country. The role requires a strong understanding of SNF regulations and experience in consulting with social workers regarding day-to-day operations and compliance with social work processes. The LMSW Consultant will be responsible for providing professional social services that conform to standards set by regulatory agencies, ensuring the quality and necessity of services provided. The consultant must maintain open communication with the Director of Social Work, providing written documentation of services rendered and making recommendations for improvements. The position requires adherence to facility policies and procedures, and the consultant must be available for consultation and coordination of social services activities with various departments, including administration, medical, and nursing services. In addition to administrative duties, the consultant will assist in planning and implementing social services programs, interpreting residents' social, psychological, and emotional needs to the care team, and providing information to residents and their families about available support agencies and financial assistance programs.
