About The Position
The Care Coordinator will be responsible for developing a long-term relationship with our patients to effectively enroll them in the Link program or refer them to a participating provider. This role will serve as a communication liaison for the Territory Manager, Physician Office, and Patient. The expectation is that they will be accountable for ensuring the patient's enrollment is successful and all the supporting documentation is completed in a timely manner to allow the patient to receive supplies.
