The Life Skills Clinician provides therapeutic and skill-building services to support individuals in developing the practical and emotional competencies necessary for independent and stable living. This role delivers services in multiple settings including the clinic, community, and patients’ homes, helping individuals apply treatment goals in real-world environments. The Life Skills Clinician focuses on strengthening daily living skills, coping strategies, and community functioning while collaborating with the interdisciplinary care team to support recovery, stability, and overall well-being.

Requirements

Strong working knowledge of social determinants of health and the challenges facing underserved and low-income populations
Familiarity with local community resources and social services systems
Excellent interpersonal, written, and verbal communication skills
Ability to pass a background check and drug screen in accordance with FQHC and regulatory requirements
Bachelor’s Degree in Social Work, Psychology, Human Services, or related field required or equivalent experience
Minimum of two (2) years of experience working with individuals experiencing mental health, substance use, or social needs in a clinical, community, or social services setting required
Must be credentialed according to the requirements of the evidence-based model(s) used
Valid driver’s license, and a clean or passable driving record must be continually maintained in compliance with organizational standards. This position requires regular travel throughout the service area using a personal vehicle. Mileage reimbursement is provided per organizational policy.
To perform this job successfully, an individual should have moderate computer skills, including Microsoft Office (specifically Word and Excel) and other applications.
Proficiency with electronic health records
Must be able to sit, stand, walk, and drive for extended periods and may occasionally involve lifting materials up to 25 pounds.
Life Skills Clinician will work independently in home and community settings while maintaining professional boundaries.
Work is performed in a variety of settings including a clinical office environment, patients’ homes, and indoor/outdoor community locations. The candidate must be comfortable with environments that may include pets, secondhand smoke, varying cleanliness conditions, and other home environment factors.The role may involve exposure to emotionally demanding situations, including individuals experiencing crisis or behavioral distress.

CPI (Non-Violent Crisis Intervention) certification preferred
Bilingual or multilingual fluency preferred

Responsibilities

Build therapeutic rapport with patients from diverse cultural, linguistic, and socioeconomic backgrounds
Engage patients who are difficult to reach using motivational interview and trauma-informed approaches
Partner closely with licensed behavioral health and medical providers to review care plans and identify patient-specific life skills goals
Provide individualized and group-based life skills training tailored to patient needs and treatment goals
Support patients in developing social skills, communication strategies, stress management techniques, positive health behaviors, and problem-solving abilities
Assist patients in navigating healthcare systems, public transportation, government agencies, and community organizations
Conduct home visits to assess the patient’s living environment and provide skills training in a real-world context
Deliver community-based services at schools, transitional housing sites, shelters, or other locations as appropriate
Accompany patients to appointments, job interviews, or community activities as clinically indicated to build independence
Identify community resources and establish collaborative relationships with local organizations to support patient needs
Work with families and natural supports to reinforce skills in the home environment and promote generalization across settings
Participate in care team meetings, case consultations, and supervision to ensure integrated, patient-centered

Communicate regularly with licensed behavioral health providers, primary care providers, social services, and other staff regarding patient progress, barriers, and changes in status
Assist in coordinating referrals to community resources, social services, housing, employment, and other supportive services
Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR)
Apply knowledge of protective service mandates for both adults and children
Utilize risk screens, de-escalation, and crisis intervention techniques when needed
