About The Position

The Life Skills Clinician (LSC) will engage with individuals and families to improve their health and wellness, live self-directive lives, and strive to reach their full potential. The LSC monitors and provides care coordination and life skills training and development, in collaboration with the care team, to youth and/or adults with co-occurring mental health and/or substance use disorders- including those with serious and persistent mental illness. Bachelor's Degree required.

Requirements

  • Bachelor's Degree required.
  • Ability to collaborate in a team setting.
  • Excellent communication skills.
  • Positive attitude toward problem-solving.
  • Ability to work with clients in various stages of change.

Responsibilities

  • Engage with individuals and families to improve health and wellness.
  • Provide care coordination and life skills training and development.
  • Support comprehensive psycho-social assessments as part of a multidisciplinary care team.
  • Help clients regulate emotions.
  • Document services provided in Electronic Medical Record (EMR) based on policies and procedures.
  • Transport clients in personal vehicles and transport client medications when necessary.
  • Complete timecards, PTO requests, mileage reports, and other documentation per Community Behavioral Health policies and procedures.
