About The Position

The LifeSet Specialist at Youth Villages is responsible for providing transitional living services to young adults aged 17-23, helping them become self-sufficient by utilizing community resources. This role involves case management, crisis support, and collaboration with clients to develop transition plans that support their goals and independence. The specialist will work closely with clients in both home and community settings, ensuring they have the necessary skills and resources to navigate adulthood successfully.

Requirements

  • Master's degree in social services field preferred; Bachelor's degree required.
  • One year of related experience in counseling and/or case management preferred.
  • Strong organizational skills and attention to detail.
  • Excellent written, verbal, and oral communication skills.
  • Ability to manage multiple priorities simultaneously.
  • Basic computer knowledge.
  • Valid driver's license and personal vehicle for work purposes.

Nice To Haves

  • Experience with youth in foster care or transitional living programs.
  • Knowledge of community resources available for young adults.

Responsibilities

  • Provide individual services to clients in home and community settings to assist with the transition to adulthood.
  • Help clients obtain and maintain safe and suitable housing, continue their education, and learn independent living skills.
  • Carry a blended caseload of LifeSet and Independent Living Program (ILP) youth, conducting weekly and monthly sessions as needed.
  • Provide on-call crisis support to LifeSet clients 24/7, alternating duties with other specialists.
  • Implement interventions suggested by supervisors in a timely manner and participate in weekly supervision exercises.
  • Conduct ongoing assessments of young adults' needs from a strength-focused, solution-based perspective.
  • Collaborate with youth to produce transition plans that support their goals and self-determination.
  • Attend court hearings and foster care review boards with young adults.
