About The Position
LifeLong Medical Care AmeriCorps Health Fellows will engage in various activities aimed at improving health education, care coordination, and community outreach. The position involves providing education in one-on-one or group settings, assisting individuals with health-related services, and following up as needed. Responsibilities also include case management and patient navigation, where fellows will assist individuals with the enrollment process for health programs. Community engagement and outreach efforts will be conducted to educate the community about available resources, programs, and healthcare options. Additionally, fellows will coordinate, plan, and participate in various community service projects, and assist with the recruitment, training, and management of non-AmeriCorps volunteers.
