Position Description: The Life Enhancement Liaison/Assistant works as a liaison between both the clinical nursing team and life enhancement to assist in providing life enhancement education to peers, implement engagement programs on households, provide one-to-one engagement to patients, and promote person-centered care. In collaboration with nursing and life enhancement, the Life Enhancement Liaison will assist in educating PCA’s on the household in regards to the benefits of engagement, how to engage various types of patients, and encourage nursing staff to apply their training into small household programs. They will also assist the life enhancement team during community-wide events when needed and work collaboratively to provide a high quality of life and wellness to patients. The Life Enhancement Liaison will provide support and direction to the interdisciplinary team to meet all the needs of the patients, their families and unit staff. They will strive to make every encounter with patients and families into a positive and meaningful experience and opportunity. The Life Enhancement Liaison will also participate in self managed work teams that ensure household activities are consistent with the Hebrew Rehabilitation Center mission, vision and model of care.
