Requirements
- Active California LVN license in good standing
- Graduate of an accredited vocational nursing program
- Minimum 1 year of nursing experience (home health preferred)
- Valid California driver’s license and reliable transportation
- Ability to assist and move patients safely
- Strong communication, time management, and documentation skills
- Comfort using electronic documentation systems
Responsibilities
- Deliver skilled, patient-focused nursing care based on personalized care plans
- Accurately document all visits, assessments, and treatments in the electronic health record (EHR)
- Collaborate with patients, families, and the care team to ensure excellent outcomes
- Follow all California home health regulations and company policies
