About The Position

The Role Enso Village is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our team in providing high-quality care and wellness support for our residents across Memory Care, Assisted Living, and Independent Living communities. The LVN will collaborate with our interdisciplinary team to promote holistic well-being, ensure regulatory compliance, and uphold our commitment to resident-centered care. This position fosters a culture of contemplative care, promoting well-being, dignity, and compassion for older adults.

Requirements

  • Graduate of an accredited Licensed Vocational Nursing (LVN) program.
  • Current California State Board of Nursing license.
  • First Aid/CPR certification and medication administration training required.
  • Minimum three (3) years of experience in senior living, dementia care, or a healthcare setting.
  • Strong leadership, team coordination, and mentoring abilities.
  • Ability to stay calm and composed in challenging situations while managing multiple priorities.
  • Exceptional verbal and written communication skills.
  • Critical thinking and problem-solving capabilities.
  • Deep understanding of dementia-related behaviors and compassionate caregiving approaches.
  • Knowledge of HIPAA regulations and the ability to maintain confidentiality.
  • Ability to walk, bend, stoop, and lift up to 35 pounds.
  • Exposure to potential hazards, including disinfectants, infections, and bloodborne pathogens.
  • Capability to push wheelchairs weighing between 50 and 250 pounds.
  • All candidates must undergo a background check and drug testing as part of the hiring process to ensure the well-being of our team and guests.

Nice To Haves

  • Prior experience working with individuals living with dementia or memory-related conditions is highly preferred.

Responsibilities

  • Provide hands-on assistance with mobility, daily living activities (bathing, grooming, eating), and emotional support.
  • Ensures compliance with all aspects of the medication management process across all levels of care, including administration, documentation within the health record, recordkeeping, ordering, and required reporting requirements.
  • Regularly review and update care plans to align with residents' evolving needs.
  • In the absence of the Health and Wellbeing Director, develop and implement interim service plans as care needs arise.
  • Coordinate resident appointments and transportation as needed.
  • Maintain positive relationships with residents, families, and physicians.
  • Act as a liaison between the care team and leadership, ensuring seamless communication.
  • Regularly review and update care plans as needed, ensuring that care companions and other team members are informed of any modifications or adjustments.
  • Perform household or unit-related tasks as needed.
  • Take on other duties as assigned to support residents and team operations.
  • Manage documentation and care records using MatrixCare (Electronic Health Record).
  • Ensure compliance with medication management protocols, including administration, record-keeping, and reporting.
  • Monitor resident alert status and document updates in the EHR.
  • Implement quality-of-care standards, infection control measures, and emergency response protocols.
  • Maintain a safe and clean environment, identifying potential hazards and recommending improvements.
  • Organize and oversee the daily activities of the care companion team in the absence of the Health and Wellbeing Director.
  • Ensure care companions and other team members remain informed of any modifications to care plans.
  • Assist with training and mentoring care companions, reinforcing Enso Village’s contemplative care approach.
  • Supervise team performance in the absence of the Health and Wellbeing Director, ensuring reliability and adherence to care standards.
  • Fosters a culture of contemplative care, promoting well-being, dignity, and compassion for older adults.
  • Actively participate in team meetings, education sessions, and professional development opportunities.
  • Promote a compassionate, resident-centered approach to care.
  • Contribute to the development and refinement of care policies, procedures, and training programs.
