Job Summary: CareLinx by Sharecare is looking for Licensed Vocational Nurses (LVNs) to join CareLinx by Sharecare. LVNs have the opportunity to provide care to patients in a variety of settings including skilled nursing facilities, assisted living facilities, patients' homes, senior living centers, and hospital/clinical settings. We offer various shifts including day, night, weekdays, and weekends – choose the shifts to fit your schedule. Essential Job Functions: Provide care within your scope of practice potentially including helping with the preparation, implementation and continuing evaluation of the patient care plan; collecting, recording, and monitoring patient data, routine measurements, and vital signs; and administering immunizations and therapeutic injections as ordered by providers Communicates with the care team as needed. Completes, maintains , and submits accurate and relevant documentation. Supervises ancillary personnel and delegates responsibilities when .
