About The Position
Garden City Health Care Center, a skilled nursing facility located in Modesto, CA, is currently seeking qualified and caring Licensed Vocational Nurses (LVNs) to join our nursing team. Requirements
- Current LVN license in good standing with the California Board of Nursing
- CPR certification (required)
- Ability to lift, push, and move up to 25 lbs and assist in emergency situations
- Must pass background screening and meet facility health requirements
Nice To Haves
- Experience in a skilled nursing or long-term care setting strongly preferred
- Strong documentation, leadership, and communication skills
- Knowledge of PCC (PointClickCare) preferred
Responsibilities
- Supervise and direct Certified Nursing Assistants during assigned shifts
- Oversee daily operations and ensure adherence to care plans and facility policies
- Participate in staff evaluations, orientation, training, and quality assurance initiatives
- Communicate effectively with residents, families, physicians, and interdisciplinary teams
- Respond to incidents, complete documentation, and escalate concerns to Nurse Supervisor
- Administer medications and treatments per physician orders
- Monitor and chart resident health status and care interventions
- Support admissions, discharges, and transfers with proper documentation
- Participate in care planning and discharge planning processes
- Maintain accurate records in compliance with state, federal, and facility requirements
- Enforce hand hygiene, infection control, and safety procedures
- Participate in emergency preparedness and fire/disaster drills
- Maintain a clean, safe, and sanitary work environment
- Ensure proper use of PPE and disposal of medical waste
