Primary Work Location: Natalie’s Place is one of several Cardea Health transitional housing programs for medically complex and behaviorally vulnerable unhoused individuals, located in Fremont. Cardea Health’s transitional housing programs collaborate with a wide range of referring partners across Alameda County to support individuals with complex health and social needs. Natalie’s Place provides comprehensive supportive services, including onsite nursing and caregiving support, transportation to medical appointments, nutritious meals, housing navigation and placement assistance, and additional wraparound services designed to promote stability, recovery, and long-term housing success. Job Summary: Cardea Health seeks a highly motivated Licensed Vocational Nurse with a special interest in working with people experiencing homelessness to work as a Respite LVN. The Respite LVN will be the primary point of clinical care at Natalie’s Place located in Fremont, CA. The ideal candidate will be invested in Cardea Health’s mission of serving marginalized populations with compassion and empathy. Cardea clinical staff need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience with homeless or incarcerated populations is highly desirable.

Requirements

  • Graduate from an accredited vocational nursing program
  • Hold current license as Licensed Vocational Nurse through the Board of Vocational Nursing and Psychiatric Technicians
  • Current Basic Life Support Certification

Nice To Haves

  • Two years of experience as a LVN
  • Self-directed and able to work with minimal supervision
  • Detailed oriented and highly organized
  • Prior experience with homeless or incarcerated populations is desired
  • Proficiency in Microsoft Outlook, Word and Excel

Responsibilities

  • Provide quality care in accordance with Cardea Health policies, procedures, and standards.
  • Ensure that all clients are treated with dignity and respect.
  • Execute care plans for admitted clients in collaboration with primary and specialty care teams, medical directors, and client
  • Provide clients with appropriate information and education to make informed choices related to medical care and treatment that reflects individual preferences
  • Maintain communication among clinical team across shifts
  • Communicate effectively to partner hospitals, community health clinics, homeless service providers, and other collaborating agencies
  • Communicate with referring partners regarding program referrals
  • Work with team members to develop creative solutions to meet the needs of individual clients
  • Participate in the collection and maintenance of data on program operations for reporting and analytical purposes
  • Ensure adherence to infection control standards on site among staff and residents
  • Participate in population health interventions among clients that contribute to safety and health including measures designed to stop the spread of Covid-19
  • Ensure that clinical documentation is current, timely, and accurate
  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Recognize and respond to medical emergencies, follow up with MD communication or 911 activation
  • Practice harm reduction principles, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
